To subscribe please print out the attached form and mail it to the Journal offices at the following address:

Subscriptions are $25.00 per year for domestic addresses and $30.00 per year for foreign addresses. All subscriptions will be renewed automatically unless the subscriber provides timely notice of cancellation. Claims for unreceived issues will be honored for fifteen months from the date of publication.
